Requirements:

Have a true passion for helping others, specifically the senior citizen population. Have strong interpersonal skills and can communicate frequently and effectively with the office regarding patient needs Have minimum 6 months experience as an LPN, in either a homecare or senior living setting Have a valid NY LPN license, are CPR certified, and provide 2 professional references. Have valid/unexpired Malpractice Insurance Have a valid driver's license Physical completed within the year with proof of MMR and Quantiferon/PPD.

Duties:

Provide skilled nursing care including vital signs, wound care, catheter care and medication management in a home setting. Ensure adherence to infection control, safety standards, and regulatory guidelines . Maintain accurate, timely clinical documentation in EMR systems. Be available to travel within the Bronx/Yonkers area.

Schedule:
