Job Description

Description:
The licensed social worker (LSW) provides professional support, advocacy, and social services to individuals and families in the community setting, addressing social, emotional, and behavioral health needs. The LSW will assist with supervision and coordination of care for clients receiving behavioral health and home health care services within the community setting. Conduct comprehensive assessments of clients' needs, strengths, and resources. Develop and implement individualized care plans. Provide social services to individuals and families Advocate for clients' rights and access to services Coordinate with healthcare providers, community organizations, and other stakeholders Maintain detailed and accurate client records Conduct home visits to determine eligibility for various services in accordance with regulatory requirements. Providing ongoing monitoring and oversight of services rendered under various state/federal programs. Educate clients about available resources and supports
Requirements:
Baccalaureate degree in social work, psychology, sociology, or other field related to social work One (1) year of social work experience in a health or human services related organization is preferred Licensure or able to obtain licensure by the state Must be able to travel to prospective clients' residences Must have a car with required insurance coverage and a state driver's license Strong interpersonal and communication skills Ability to handle sensitive and confidential information Demonstrated ability to work both independently and as part of a team Strong organizational skills and attention to detail Compensation Competitive hourly rate based on experience Comprehensive Benefits Package Supportive team environment Flexible scheduling Mileage reimbursement Ongoing supervision and professional support
