What you'll do Perform skilled nursing visits under the supervision of the RN Case Manager and established plan of care Administer medications, injections (including B12 and insulin), and IV therapy within LVN scope Provide wound care, foley catheter changes, ostomy care, and diabetic management Monitor vital signs and patient response to treatment; report changes to the RN Educate patients and families on disease process, medication compliance, and home safety Document accurately and timely in Axxess EMR (training provided) Coordinate with the RN Case Manager, therapy team, and HHAs to ensure continuity of care Participate in case conferences and QAPI initiatives Must-haves Active, unencumbered Texas LVN license Current CPR/BLS certification Valid Texas driver's license, auto insurance, and reliable transportation Ability to lift 50 lbs and perform hands-on patient care in the home setting Comfort working independently in patients' homes with RN backup by phone Nice-to-haves 1+ year of home health, hospital, SNF, or clinic nursing experience (new grads welcome to apply — we offer structured onboarding) Bilingual English/Spanish (strongly preferred — most patients are Spanish-speaking) Experience with Axxess EMR IV certification Wound care training Why LVNs choose EPHH LVN-to-RN bridge support — we actively support team members pursuing their RN through tuition assistance and flexible scheduling Joint Commission accreditation — you'll work in a clinically rigorous, well-run agency Real RN mentorship — our RNs are experienced and available, not absent Stable caseload and fair productivity expectations Bilingual, family-oriented team culture Career growth pathways into wound care, IV team, or case management
