Job Summary:

The Licensed Vocational Nurse (LVN) provides direct patient care to residents in a nursing home setting, ensuring their health and well-being. The LVN works under the supervision of a Registered Nurse (RN) and collaborates with the healthcare team to deliver high-quality care tailored to the individual needs of residents.

Key Responsibilities:

Patient Care:

• Administer medications and treatments as prescribed by the physician.
• Monitor and document residents' vital signs, health status, and responses to treatments.
• Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, and feeding.

Assessment and Monitoring:

• Conduct initial assessments and ongoing evaluations of residents' health conditions.
• Identify changes in residents' conditions and report findings to the supervising RN or physician.
• Participate in care planning and implementation of individualized care plans.

Collaboration:

• Work closely with RNs, physicians, and other healthcare professionals to coordinate care.
• Communicate effectively with residents, families, and staff regarding care plans and resident needs.

Documentation:

• Maintain accurate and timely documentation of patient care activities in compliance with facility policies and state regulations.
• Ensure that all documentation reflects the care provided and any changes in residents' conditions.

5.

Infection Control:

• Adhere to infection control protocols and safety standards to maintain a safe environment for residents and staff.
• Assist in maintaining cleanliness and organization in resident care areas.

6.

Education and Support:

• Provide education and support to residents and their families regarding health conditions, medications, and care plans.
• Assist in training and mentoring nursing assistants and other support staff as needed.

7.

Compliance:

• Ensure compliance with all state and federal regulations governing nursing home operations.
• Participate in quality improvement initiatives and staff meetings as required.

Qualifications:

• Current Texas LVN license.
• Completion of an accredited vocational nursing program.
• Previous experience in a long-term care or nursing home setting preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a team-oriented environment and manage time efficiently.

Working Conditions:

• This position may require standing for extended periods, lifting, and moving residents as needed.
• Must be able to work flexible hours, including weekends and holidays as required.
