Job Description
Job Summary:
The Licensed Vocational Nurse (LVN) provides direct patient care to residents in a nursing home setting, ensuring their health and well-being. The LVN works under the supervision of a Registered Nurse (RN) and collaborates with the healthcare team to deliver high-quality care tailored to the individual needs of residents.Key Responsibilities:
1.Patient Care:
- Administer medications and treatments as prescribed by the physician.
- Monitor and document residents' vital signs, health status, and responses to treatments.
- Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, and feeding.
Assessment and Monitoring:
- Conduct initial assessments and ongoing evaluations of residents' health conditions.
- Identify changes in residents' conditions and report findings to the supervising RN or physician.
- Participate in care planning and implementation of individualized care plans.
Collaboration:
- Work closely with RNs, physicians, and other healthcare professionals to coordinate care.
- Communicate effectively with residents, families, and staff regarding care plans and resident needs.
Documentation:
- Maintain accurate and timely documentation of patient care activities in compliance with facility policies and state regulations.
- Ensure that all documentation reflects the care provided and any changes in residents' conditions.
Infection Control:
- Adhere to infection control protocols and safety standards to maintain a safe environment for residents and staff.
- Assist in maintaining cleanliness and organization in resident care areas.
Education and Support:
- Provide education and support to residents and their families regarding health conditions, medications, and care plans.
- Assist in training and mentoring nursing assistants and other support staff as needed.
Compliance:
- Ensure compliance with all state and federal regulations governing nursing home operations.
- Participate in quality improvement initiatives and staff meetings as required.
Qualifications:
- Current Texas LVN license.
- Completion of an accredited vocational nursing program.
- Previous experience in a long-term care or nursing home setting preferred.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a team-oriented environment and manage time efficiently.
Working Conditions:
- This position may require standing for extended periods, lifting, and moving residents as needed.
- Must be able to work flexible hours, including weekends and holidays as required.
