Duties Provide direct patient care by assisting with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts and other assistive devices. Monitor patient health by observing vital signs, recording medical documentation accurately in Electronic Health Record (EHR) systems, and reporting changes to healthcare professionals promptly. Administer medications following prescribed protocols while maintaining strict adherence to HIPAA regulations to protect patient privacy. Support patients with specialized needs including dementia care, Alzheimer's care, memory care, hospice support, and working with individuals with developmental disabilities or disabilities. Assist with medical procedures such as catheter care, wound management, and helping with medical equipment like oxygen or ventilators. Prepare nutritious meals tailored to dietary restrictions and assist with feeding as needed; handle household chores including cleaning and laundry to maintain a safe living environment. Document all patient interactions thoroughly in medical records and ensure compliance with healthcare standards and safety protocols. Skills Proven experience in home health or long-term care environments, including assisted living or nursing home settings. Strong knowledge of medical terminology, medication administration, and patient observation techniques. Skilled in working with individuals with dementia or cognitive impairments requiring memory care or hospice support. Proficiency in EMR/EHR systems for accurate documentation; familiarity with HIPAA regulations for safeguarding sensitive information. Ability to operate medical equipment such as Hoyer lifts, blood pressure monitors, and other vital signs measurement tools. Excellent caregiving skills combined with patience, empathy, and a positive attitude toward senior care and patients with disabilities. Physical capability to assist with mobility tasks and perform caregiving duties safely; CPR certification is required or preferred.
