Open to local or travelers PM and Day Shifts Available Also available - weekend only opportunities 8 or 13 week opportunities Responsibilities Conduct thorough patient assessments, including vital signs, physical examinations, and health histories to develop individualized care plans. Administer medications and treatments accurately. Monitor residents' conditions closely. Collaborate with interdisciplinary teams to coordinate discharge planning and case management. Maintain detailed medical documentation. Provide education and support to residents and their families regarding health coaching, medication administration, and disease management strategies.
