Job Objective:

Coordinates delivery of nursing and social services for adult day participants Minimum Qualifications

Education:

Graduate of accredited nursing school

Experience:

2-3 years of nursing experience. Experience working with the elderly and those with dementia/Alzheimer's Disease, preferred work in a community-based setting and knowledge of services and entitlements for frail elderly are a plus.

Licensure:

State of Connecticut Nursing License; CPR certification Job Standards 1. Demonstrates clear, compassionate, and effective nursing, demonstrating excellent judgement. 2. Ability to travel to client homes and other community sites as necessary related to networking and intakes. 10. Availability of personal vehicle for work related travel (mileage reimbursed). Travels primarily within Fairfield and New Haven counties. 11. All other related duties as assigned. Essential Job Functions 1. Conduct new client intakes and assessments to determine the appropriateness of referrals and encourage participation at start of program and once enrolled. 2. Conduct/assess the nutritional, medical, physical and mental functional needs of the clients during the initial assessment and ongoing as necessary within scope of nursing practice.
