Job Description

What you'll do The LPN Clinic Coordinator provides direct and indirect patient care under the direction of the Registered Professional Nurse or Physician. The LPN functions as part of the interdisciplinary health care team. The LPN Clinic Coordinator provides direction for daily nursing assignments and acts as the liaison between the medical staff and the nursing staff regarding clinical issues. Works with the Nurse Team Leader, Supervisor, and/or Manager to assure the appropriate application of the nursing process Assists in the orientation, training and documentation of new clinical staff according to random sampling of personnel records by supervisor Works directly with the team leader, supervisor and or manager to assess and meet continuing education needs and requirements for self and nursing staff, as identified through observation Functions in a staff nurse capacity at least 50% - 75% of time and as needed to ensure quality patient care and safety, as observed by supervisor Works directly with team leader, supervisor, and the manager to implement Performance Improvement and Risk management activities and practices, as observed by supervisor or manager As delegated, makes clinical assignments, monitors performance, sets work schedules and arranges coverage Post schedules at least four to six weeks in advance, as observed by manager/supervisor/team leader Monitors workload, shifts staff as needs, as observed by manager/supervisor/team leader Independently solves staffing problems, keeps manager/supervisor/team leader informed of situations beyond his/her resources, as observed by manager/supervisor Provides input for evaluations of LPN's in clinic as requested on time with 100% compliance, as reviewed by manager/supervisor/team leader Assists with interviewing candidates for open nursing positions giving useful feedback to the supervisor/manager/team leader in requested timeframe Assists in the development of proposals for improved efficiency, productivity or expansion of nursing services, offers workable suggestions with follow through, as observed by team leader/supervisor/ manager Monitors time and attendance of LPN's in clinic and addresses issues as they arise in conjunction with team leader/ supervisor/ manager Assists in the development of effective interdepartmental working relationships, to ensure smooth, efficient patient care systems Sets an example in work performance and team building attitudes, concern for patients and support of clinicians by maintaining a positive interpersonal attitude and demeanor, as observed by manager/supervisor/team leader Facilitates meetings/huddles with staff, during provider educational time, to discuss patient and clinic concerns. Meeting notes or debrief provided to team leader, supervisor, or manager in a timely manner Works with clinical leadership to assure compliance with all outside accrediting and regulatory agency codes and standards, as observed by manager/supervisor/team leader Utilizes property and supplies appropriately and prudently to minimize waste without sacrificing quality Assists in the regular purchasing practices of clinic supplies Regularly assesses the par level of supplies in use to assure supply availability Serves as a staff resource to the clinic manager/supervisor/team leader, providers and other department members for problem solving and information gathering Consistently ensures that patient information remains private and confidential, by identifying privacy issues and sets policies to ensure the protection of patient information, as observed by manager/supervisor/team leader Does not discuss patient information except as necessary in the performance of job, as observed by manager/supervisor/team leader Provides input to the Clinical leadership regarding potential issues surrounding Nursing Policy and Procedure Disseminates nursing perspective and information to clinical leadership and providers and other department members as needed for problem solving Meets with team leader/ supervisor/manager at regular intervals, at least twice a month, to share staff concerns as related to patient care issues Maintains current licensure, certifications, and competencies Provides proof of current BLS validation to RN Manager Completes required unit credentialing according to departmental policy by providing documented evidence of completion to RN Manager Provides to RN Manager evidence of completion of PC-based age-specific competency education for targeted age groups within the previous calendar year Participates in 12-18 hours of educational activities annually Performs similar or related duties as requested or directed Performs other duties as requested and observed by team leader, supervisor, and or manager Performs and documents Point of Care (POC) testing and quality control in accordance with established laboratory policies and procedure as observed by manager and POC staff Maintains and documents testing reagents/supplies and equipment according to established laboratory procedures as observed by manager and POC staff What we need from you
Education:
Graduation from a Practical Nursing program, required
Experience:
Minimum 2 years nursing experience at Bassett Healthcare in current practice, required
Licensure/Certifications:
Current NYS licensure as a Practical Nurse, required BLS Certification within 6 months of hire, required

Bonus Amount:
RN (excluding management) sign-on bonuses are awarded in the amount of $20,000 for full time LPN sign on bonuses are awarded in the amount of $10,000 for full time

Fox Hospital, Bassett Medical Center, Cobleskill Regional Hospital, Little Falls Hospital, O'Connor Hospital, Valley Health Services and Valley Residential Services Pay Transparency Salary is based on a variety of factors, including, but not limited to, qualifications, experience, education, licenses, specialty, training, and fair market evaluation based on industry standards. The above compensation range represents a good faith belief of the compensation range by Bassett Healthcare Network, and its entities and affiliates, at the time of this posting or advertisement.
Pay Range Minimum:
USD $25.73/Hr.
Pay Range Maximum:
USD $38.69/Hr.
