JOB STANDARDS
The Licensed Practical Nurse must have: A license to practice in the Home's state as a Licensed Practical Nurse. The ability to function as the nurse in charge during their assigned shift. Possess good written and verbal communication skills, including the ability to read, write and understand English. Be able to function independently and perform related duties/tasks with a minimum of supervision or direction. Good observational skills and a working knowledge of the nursing process. The ability to supervise others and/or have prior management experience. The necessary leadership skills to interact effectively with staff, Residents and families. The ability to perform •essential job functions with or without reasonable accommodations.
JOB RESPONSIBILITIES AND DUTIES
The Licensed Practical Nurse will: Observe and monitor assigned Residents daily and report current status to the relief nurse. Timely reporting of changes in Residents status or condition to Director of Nursing or their designee. Notify Resident physician and responsible party of change of Resident status, with prior reporting (if possible) to Director of Nursing or Mother Superior. Assist Resident with issues related to psychosocial needs, i.e. aging and health changes/illness, loss and grief. Administer and document prescribed medications/treatments accurately and timely and in compliance with policies/procedures. Respond calmly and quickly to emergencies involving Residents or employees and report the situation to the Director of Nursing or her designee. Transcribe accurately and timely all physician orders. Participate in the comprehensive Resident assessment and update the plan of care in a timely manner to assure its accuracy and appropriateness. Prepare for and assist with Physician visits to the Home. Assist with orientation of Nursing Assistants and/or select nursing personnel as indicated. Provide necessary supervision and follow up of care performed by Nursing Assistants. As required, participate in the performance evaluation of Nursing Assistants. Report to the Little Sister, Director of Nursing, or her designee any unresolved or pertinent issues with Nursing Assistants. Assure follow up of rehabilitation/restorative measures. Communicate daily all pertinent Resident care information to Nursing Assistants, Nursing Staff, and the Director of Nursing. Communicate with family/responsible party regarding Resident issues/status as indicated with the assistance of the designated Little Sister of the Poor and/or Director of Nursing. Prepare accurately and timely documentation, including but not limited to accident/incident reports, admission/readmission, discharges, changes in Residents conditions/status, etc. Monitor medical supplies and equipment to ensure proper use and safety. Strive for optimal use of supplies/materials, keeping in mind cost and budget limitations Maintain confidentiality of all Resident, departmental and organizational information. Protect Residents rights and personal healthcare information. Work effectively and cooperatively as a team member within the department and with all departments or other disciplines. Have the desire to work in the area of geriatrics, rehabilitation, and/or psychiatric nursing. Acquire the basic knowledge of State and Federal regulations and guidelines governing long term care. Observe dress and conduct codes, including wearing identification badge. Meet attendance standards as established and provide proper notice of absence. Complete all assignments and duties in prescribed time. Follow all safety, security, infection/exposure control and hazardous waste policies and procedures (including Category I universal precautions). Attend required in-services and department meetings to continually refine clinical skills. Perform other duties as assigned by supervisor. Offer flexibility with the schedule to meet the needs of the Nursing Department.
