Job Description

Active Licensed Practical Nurse (LPN) license and minimum 1+ years Home Health Licensed Practical Nurse (LPN) experience required. Applicants who do not meet these qualifications will not be considered. Provide compassionate, patient-centered nursing care as a Licensed Practical Nurse (LPN) in home health, delivering skilled visits to individuals in their homes within the Roseburg, OR area. This full-time, contract position involves managing a caseload of approximately 3-5 patient visits daily (25-32 points per week) Monday through Friday, 8:00 AM - 5:00 PM, with occasional schedule flexibility to meet patient needs. Reliable transportation is required for travel within the assigned territory, with typical mileage reimbursement.
Key Responsibilities:
Conduct skilled nursing visits to assess, plan, and implement individualized care plans Administer medications, provide wound care, and manage IV therapy or other ordered clinical interventions Monitor patient conditions, promptly reporting changes to RN Case Managers or physicians Educate patients and caregivers on disease management, medication administration, and self-care strategies Accurately document assessments and care in the Home Care Home Base (HCHB) system Collaborate with interdisciplinary healthcare team members to coordinate care and ensure compliance with regulatory standards Maintain infection control and patient safety protocols Participate in case conferences, quality improvement, and required training
Qualifications:
Active, unrestricted LPN license in Oregon Minimum one year of home health, hospice, or community-based care experience Proficiency in Home Care Home Base (HCHB) electronic charting Valid driver's license, reliable insured vehicle Current BLS certification Strong clinical judgment, time management, and communication skills Ability to work independently in the field managing a dynamic patient caseload
Preferred:
Experience with Medicare-certified home health agencies Comprehensive knowledge of home health compliance and documentation standards Exceptional interpersonal skills and patient-focused care approach
