The Springfield Center for Living and Rehabilitation, a premier residential, long-term, rehabilitation, and subacute care provider, is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team. As an LPN at our facility, you will provide exceptional patient care and support under the supervision of our experienced nursing staff. Responsibilities:
Provide direct patient care to residents in accordance with their care plans and under the supervision of licensed nursing staff Administer medications, treatments, and vaccinations as prescribed by physicians and as needed Monitor and report changes in resident condition, including vital signs, weight, and treatment response Collaborate with other healthcare professionals to develop and implement individualized care plans Participate in quality improvement initiatives to ensure optimal care and patient outcomes Maintain accurate and thorough documentation of patient care and treatment Engage with residents, families, and caregivers to promote a positive and respectful environment
Requirements:
