$1,694.88 Weekly Position Summary We are seeking a dedicated Licensed Practical Nurse (LPN) for a Long Term Care (LTC) assignment in Santa Fe, NM. The ideal candidate will provide high-quality nursing care to residents in a skilled nursing and long-term care environment while collaborating with the interdisciplinary healthcare team to support patient safety and well-being. Responsibilities Provide direct nursing care to long-term care residents in accordance with individualized care plans Administer medications, treatments, and nursing procedures as prescribed Monitor resident conditions and promptly report changes to the supervising RN or physician Maintain accurate and timely patient documentation Assist with activities of daily living (ADLs) and resident support needs Promote a safe, clean, and therapeutic environment for residents and staff Collaborate effectively with nursing staff, providers, therapists, and support personnel Follow infection control, safety, and facility compliance standards Deliver compassionate, patient-centered care during night shifts Qualifications Active New Mexico LPN license or active multistate compact license required Current AHA BLS certification required Minimum of 2 years of recent LPN experience required Previous Long Term Care (LTC) or Skilled Nursing Facility (SNF) experience preferred Strong communication, clinical, and organizational skills Ability to work independently and as part of a healthcare team Commitment to delivering compassionate and professional patient care For more details reach at or Call / Text at 516-587-6677.
