LPN or RN for Private Duty

Community Home Health Care (formerly Accu Care Home Health) has been specializing in providing Private Duty Nursing for both Pediatric and Adult clients for over 35 years in the Capital District and surrounding Counties. We are looking for two LPNs or RNs in Rensselaer, NY, for a patient in need of

OVERNIGHTS.

Patient Info:

24-year-old: legally blind, non-verbal/ambulatory O2, G-tube, suctioning, nebulizer Vent/Trach - must be confident in changing trach ties.

Hours:

Overnights 10 or 12-hour shifts Responsibilities Provide direct patient care in a home setting, including catheterization, IV insertion, and tube feeding. Conduct thorough patient assessments and maintain accurate electronic health records (EHR). Administer basic life support and CPR as needed, ensuring patient safety at all times. Collaborate with healthcare teams to develop and implement individualized care plans. Educate patients and families on health management and home care techniques.
