Colby, KS Overview We are seeking a dedicated Licensed Practical Nurse (LPN) specializing in Long Term Care to join our healthcare team. In this vital role, you will provide comprehensive nursing care to residents in a long-term care setting, ensuring their comfort, safety, and well-being. The ideal candidate will possess a strong foundation in patient assessment, medication administration, and chronic disease management, with a commitment to delivering compassionate, high-quality care. This position offers an opportunity to make a meaningful difference in the lives of individuals requiring ongoing support and medical attention. Duties Perform thorough patient assessments, including vital signs, physical examinations, and health history documentation. Administer medications and injections accurately following physician orders while adhering to HIPAA regulations and safety protocols. Monitor residents' conditions closely, recognizing signs of acute pain, infection, or deterioration requiring prompt intervention. Manage complex care needs such as IV infusion therapy, catheterization, tube feeding, and ventilator support. Document all patient interactions and treatments meticulously within electronic health record (EHR) systems like Epic or Cerner. Collaborate with interdisciplinary teams to develop and implement individualized care plans focusing on dementia care, Alzheimer's care, memory care, and behavioral health needs. Assist with discharge planning and coordinate with family members or guardians to ensure continuity of care. Maintain infection control standards through aseptic techniques and proper sterilization procedures. Provide education on health coaching topics including nutrition, medication management, and chronic disease self-care. Support emergency response efforts by participating in triage assessments and critical incident management. Requirements Valid licensure as a Licensed Practical Nurse (LPN) in the respective state or jurisdiction. Proven experience working in long-term care facilities such as nursing homes or assisted living communities. Demonstrated knowledge of medical terminology, anatomy, physiology, and ICD coding systems (ICD-9/ICD-10). Experience with EMR/EHR systems like Epic, Cerner, or Athenahealth for accurate documentation. Familiarity with specialized care areas including hospice care, geriatrics, dementia/memory care, and acute pain management. Ability to perform venipuncture, catheter care, sterile processing techniques, and basic life support (BLS). Strong assessment skills with the ability to recognize changes in patient condition promptly. Excellent communication skills for effective collaboration with healthcare teams and families. Knowledge of infection control protocols and HIPAA compliance standards. Prior experience with IV insertion, medication administration routes (including injections), and specimen collection is preferred. Willingness to participate in ongoing training related to trauma centers (Level I/II), emergency medicine practices, or pediatric/neonatal ventilator management is advantageous.
