Job Description:

As an LTSS Care Coordinator, you will be responsible for: Assessing the needs of individuals requiring long-term care services. Developing, implementing, and monitoring care plans. Coordinating with healthcare providers, social workers, and other stakeholders. Advocating for the patient's needs and rights. Ensuring compliance with healthcare policies and regulations. Providing education and resources to patients and their families. Documenting all encounters and relevant activities.

Qualifications:

Ability to effectively communicate in the Spanish language. Bachelor's degree in Nursing, Social Work, or a related field. Minimum 2 years of experience in healthcare, social work, or case management. Strong understanding of long-term care services and systems. Excellent communication and interpersonal skills.
