We are seeking an experienced Hospital Social Work leader with a strong background in acute care and case management leadership. This role is ideal for someone who brings clinical expertise, leadership experience and a passion for care coordination across the continuum. This role partners with physicians, leadership and community partners to improve patient outcomes, enhance care coordination and support high quality, patient centered care.
