Opens a new window. Description Los Angeles Community Health Project (CHPLA) is a program of Heluna Health. CHPLA's mission is to improve the health and wellbeing of people affected by substance use in Los Angeles, to increase access to health care, empower people to protect themselves, and educate each other to reduce harm in our communities. CHPLA combines direct services, advocacy, outreach, education, and research collaborations to fulfill this mission. Los Angeles Community Health Project was founded in 1992. Law Enforcement Assisted Diversion (LEAD) is a community-based diversion approach with the goals of improving public safety, and reducing unnecessary justice system involvement of people who participate in the program. In a LEAD program, law enforcement officers exercise discretionary authority at point of contact to divert individuals to a community-based, harm-reduction intervention for law violations driven by unmet behavioral health needs. In lieu of the traditional criminal justice system cycle- booking, detention, prosecution, conviction, incarceration
- individuals are instead referred into a trauma-informed, harm reduction intensive case-management program where the individual receives a wide range of support services.
ESSENTIAL FUNCTIONS
Communicates and coordinates with participants, case managers, motel staff and other stakeholders as needed to facilitate the timely placement of participants in temporary housing. Collects invoices, receipts and/or other required documentation from motel staff or LEAD Case Managers, and works collaboratively with Heluna Health's Accounting team to ensure timely processing of payments. Collects Sign In/Out logs within 72 hours of participant checkout. Work with theCHLPLA LEAD
team on timely coordination and collection of documentation and invoices. Maintain consistent communication with motel staff and LEAD Case Managers May be required to support LEAD Case Managers in the field. Responsible for data entry, data maintenance and compiling data for monthly reports required by funders. Become aware of all harm reduction supplies and services offered to the community through the SEP Program and be able to clearly communicate related information to stakeholders, funders, community members, peers and staff. Report essential programmatic information and developments to in a clear, organized and time effective manner.NON-ESSENTIAL FUNCTIONS

Education/Experience High School Diploma or equivalent preferred. WORK ENVIRONMENT
Most time spent in general office setting, indoors and temperature controlled. Field work takes place in communities across LA County in a range of settings, including homeless encampments, under freeways overpasses, riverbanks, parks and on partnered providers premises. Funding is by LA Care's Generating African American Infant & Nurturers' Survival Initiative (GAAINS II). This is a hybrid virtual and in-person position located in Los Angeles County, California. Applicants must reside in Los Angeles County, California. This project will provide postpartum doula in-home supportive services and support by one (1) Certified Postpartum Doula for eight (8) African American mothers and infants through a home visitation program for clients and their infants from two days postpartum through three months postpartum. Families will be able to have up to 24 hours of postpartum care in their homes (4-hour or 8-hour sessions) free of charge if they meet eligibility criteria (African American, residing in Los Angeles County, and have Medi-Cal). Breastfeeding will be promoted by doulas, with at least 70% (7 of 10) of clients, using this feeding method for a minimum of six months. Also, assessment for perinatal mood and anxiety disorders (PMADs) will be conducted and referrals shall be made for mental health services if needed. About 4-6 hours a week during Monday-Fridays from 9:00 am to 5:00 pm will be spent on outreach to identify and enroll eligible clients for the program through social media marketing on Instagram, Twitter and Facebook, as well as participating in community meetings in-person, doing doctor's office and clinic visits and participating virtually over Zoom or Teams meetings with partner agencies. Occasional outreach at health fairs on weekends is also required. Reliable transportation and personal computer are required. Interested applicants please submit a resume highlighting relevant work as a graduate of a Postpartum Doula Training Course, or perinatal home visitor. This is a temporary part time position (12.5/hours week) funded through June 30, 2026.ESSENTIAL FUNCTIONS
Understanding of quality-of-life issues and the impact of social determinants of health and racial justice issues impacting birth outcomes and survival rates of African American mothers and infants. Ability to provide culturally congruent services to African American mothers and their nurturers or fathers of the baby and make referrals to African American fathers to our Fatherhood Support Program Coordinator. Work with Program Director to implement a plan to enroll eight (8) African American mothers and their infants who are between two days and three months postpartum into the program provide in-home supportive services during the two-day to three- month postpartum period. Under supervision of Program Director, implement strategies to ensure at least 70% (5 out of 8) of African American mothers participating in the program will breastfeed their babies for a minimum six months during the child's first year of life. Implement plan to deliver strategies to ensure at least 95% (7 out of 8) of African
- American mothers and their babies participating in the program will survive through one year post birth.
JOB QUALIFICATIONS
Graduate of a Postpartum Doula Training or Birth Doula Training having worked with minimum three mother-baby dyads during the first 8 weeks after birth; Certified Postpartum Doula preferred. Certification as a Lactation Educator or Lactation Counselor-Educator or Breastfeeding Peer Counselor through any qualified organization such as CAPPA, Breastfeed LA, Le Leche League, WIC, UCLA, etc. preferred; Six months experience working as a postpartum doula or in-home health visitor providing social services to a diverse population including Black/African American families. Demonstrated ability to implement research-based health education strategies and best practices in perinatal health and lactation. Ability to plan, coordinate, and organize work projects and meet deadlines. Strong interpersonal communication skills and conflict resolution ability. Cultural awareness training and anti-racism training or willingness to attend training within 1 month of hire. Experience collaborating with a variety of staff at all levels and community partners. Strong attention to detail to complete online visit forms by deadlines; Passion for and commitment to the mission of Happy Mama Healthy Baby Alliance Aptitude for solving problems with creativity and resourcefulness. Ability to develop positive relationships with clients, doulas, providers, staff, interns, apprentices, volunteers, and supervisors. Ability to work independently with little supervision, keep up with required documentation and reports and meet deadlines for submitting reports. Ability to work flexible hours (including nights, weekends, holidays), work virtually, and have reliable transportation and the ability to travel throughout Los Angeles County. Completion of HIPAA Training within 1 week of hire required. Training in Perinatal Mental Health required or willingness to be trained within 1 month of hire required. Computer proficiency with Microsoft 360 Programs such as Word, SharePoint, Excel, PowerPoint, and possession of a personal computer are required. Education/Experience High School Degree (required); or associate's degree (preferred) or college-level coursework in subject areas of women's studies, health sciences, public health, social work, family studies, obstetrical and postpartum nursing, or midwifery. Demonstrated experience in postpartum care of mothers (experience via parenting and/or early learning and development and home visitation programs is highly desirable) as a Postpartum Doula and/or Midwife Assistant. Six months experience working as a Postpartum Doula or in-home visitor providing social services and lactation support to diverse populations including BIPOC, refugees and immigrants, including adolescents. ESSENTIAL FUNCTIONS
§ Supervision of adult homeless clients in an emergency shelter in terms of access to safe housing, meals and services. § Intake of adult applicants to assess housing need, support and benefits services. § Develop and manage emergency shelter day-to-day services. § Supervision of engagement activities designed to build a healthy and safe environment for sleeping, eating, and living in a group setting. § Communication, both verbal and written, with adult clients, staff, law enforcement and others to determine situational shelter needs. § Documentation in case records, and files to ensure the written record of work performed on behalf of clients, including completion of incident reports. § Light housekeeping/general clean up. § Ability to handle emergency incidents, including medical, mental health, violent and/or other unanticipated events, including evacuation due to earthquake, chemical release, incarceration, or other incidents. § Conducts and documents hourly site walkthroughs inside and outside to ensure a safe and comfortable living safe for staff and residents § Always keeps a clean and welcoming facility by doing light housekeeping/general clean-up of all inside and outdoor spaces; stocks up program supplies as needed § Reports and seeks immediate assistance during any unsafe or unusual occurrences; in the absence of onsite management the Shelter Specialist contacts on-call management during evening and weekend hours § Attend all mandatory meetings and trainings § Other duties as requiredJOB QUALIFICATIONS
