The Quality Assurance UIR Coordinator will be responsible for ensuring ISP and PCPT accuracy and timeliness and overseeing the incident management processes and investigations
QUALIFICATIONS
BA or BS in human services, social work, psychology, or related field and excellent communication and organizational skills Proven experience in quality assurance, compliance, and data tracking systems In-depth understanding and training in investigations and compliance regulations as per DDD, NJDHS, HIPAA, and Medicaid Detail-orientated and highly organized, with the ability to manage multiple priorities and meet deadlines

RESPONSBILITIES
ISP & PCPT REVIEW Receive, track, and monitor ISP and PCPT revision requests Review revision requests to identify and document all required updates, including: Services and supports Desired outcomes and goals Health and safety needs Staffing requirements Medical and behavioral considerations Individual preferences Other person-centered information Review revised ISP and PCPT drafts Compare revised drafts against original revision requests to verify that all requested changes have been accurately incorporated Identify omissions, inaccuracies, inconsistencies, incomplete information, or areas requiring clarification Communicate review findings to the management team and ensure follow-up with Support Coordinators as necessary Maintain documentation of review findings, pending items, completed revisions, and ongoing follow-up activities Support communication between Administration, management staff, and Support Coordinators to facilitate timely completion of revisions Track outstanding revisions and continue follow-up efforts until all required updates have been completed Confirm when ISP and PCPT drafts appear complete and are ready for final management review and approval Performs other duties as assigned by Executive Director
UIR & RISK MANAGEMENT
Under the supervision of the Director of QA, serve as the primary point of contact for receiving verbal and written reports of incidents involving service recipients Ensure immediate and appropriate actions are taken to protect the health, safety, and welfare of individuals involved in incidents; direct additional actions as needed Lead investigations or assign qualified investigators for reportable and non-reportable incidents, as appropriate Ensure all reportable incidents are submitted electronically through designated web-based systems within required timeframes, including when reporting deadlines require weekend or holiday work Act as a liaison with county and regional risk management personnel regarding incident reporting and follow-up activities Collect, maintain, and organize incident reports and related documentation for tracking, analysis, and quality improvement purposes Monitor investigation timelines and ensure completion of corrective actions and follow-up requirements Generate incident management reports and data summaries for organizational use and regulatory review Analyze incident trends and identify quality indicators to support organizational quality improvement efforts Develop and recommend corrective action strategies designed to reduce incident frequency and improve outcomes Collaborate with Residential and Day Habilitation Managers and other leadership staff to implement quality improvement initiatives and risk reduction strategies. Monitor implementation efforts and evaluate effectiveness through data collection and analysis Prepare and present incident analysis, trend reports, and recommendations during Management Meetings Generate recurring reports, including but not limited to: Monthly Reportable Incident Summaries Monthly Non-Reportable Incident Summaries Trend Analysis Reports Quality Improvement Reports Assist with quality management processes and performance improvement initiatives Promote continuous quality improvement projects and ongoing performance measurement activities
