Job Description
Provides administrative support activities to the Quality department. Under direct supervision of the Quality Assurance Manager responsible for performing a variety of clinical support functions for the outpatient setting including patient outreach, care gap analyses, and patient follow-up. Maintains documents and records according to the state and federal health (HIPAA) care standards.
DUTIES AND RESPONSIBILITIES
1. Researches and complies information in a timely and effective manner to assist in the preparation, dissemination, and presentation of the program and annual evaluation of the clinical Quality Performance Improvement Plan activities. Utilizes quality care model Plan, Do, Study Act (PDSA) implement, coordinate, and monitor all programs and activities through the direction of the Quality Assurance Manager. 2. Assist the Quality staff to collect, track and perform internal and external clinical audits and/or analyzes data reports. Assist the Manager to prepare and implement processes to achieve improved performance. 3. Gathers, enters, and/or updates data to maintain departmental records. Accurate and timely data entry into databases as appropriate; establishes and maintains files and records for the program. Present summary reports and trend data analyses of program activities on a regularly scheduled basis to the Quality Assurance Manager. 4. Provides excellent internal/external customer service via telephone, fax or face-to-face contact to assist staff and patients with their health care needs and services as needed. 5. Serves as a primary point and liaison with departments in generating and/or coordinating the development of various reports regarding performance and quality outcomes and prepares responses as appropriate with the approval of the Quality Assurance Manager. 6. Identifies and follows up on care gaps in care that cannot or were not closed in the clinic. Performs patient outreach to schedule Post ED Visits and Admissions and Follow up appointments to fill gaps in care. 7. Performs other related duties as assigned.JOB QUALIFICATIONS
High School graduate or equivalent required Quality Improvement Specialist or Medical Assistant experience preferred Bilingual (English/Spanish) is preferred Computer literacy with extensive knowledge of spreadsheets and graphs Must have reliable transportation to perform clinical site audits within Bexar, Kendall and Hays counties Scheduled hours and/or work locations are subject to changePHYSICAL ACTIVITIES AND REQUIREMENTS
