The individual must have training and experience in providing hands on care/services to individuals who have a diagnosis of intellectual/developmental disability. A minimum of an Associate's degree with at least two years of documented experience working the ID/DD population is required. Individual must pass a Virginia State Police Criminal Record Check, Department of Social Services Record and FBI checks. Individual is required to complete a Tuberculosis Screening Test upon employment and every year thereafter. The Quality Assurance Specialist will have experience with organizing and managing large quantities of files and records. They will display an enthusiastic, welcoming demeanor and professional customer service skills. They will possess a minimum of two years office experience. They will be proficient with MS Office program and ability to create spreadsheets and databases with Excel. They will utilize organizational skills and the ability to self-direct. They have the ability to work well under pressure. They will have the ability to maintain strict confidentiality. They will have the ability to react to change productively and handle other essential tasks as assigned. They will have the ability to prioritize, multi-task, and produce high quality and accurate work.

Duties :

The Quality Assurance Specialist is expected to report to the Program Manager/Clinical Director and carry out all duties as assigned. They must acknowledge and act within the values of the organization. They will be supervised by the Clinical Director/Clinician. This individual will be capable of doing the following: Review of documentation submitted by direct support professionals to ensure they are in accordance with DBHDS/Medicaid standards and facilitate a systematic approach to working with clients with developmental/intellectual disability/or dually diagnosed, including working with legal representatives and other professionals. Quality Assurance Specialist will ensure direct support professional documentation encompasses the following: Life skills guidance and instructions Client intake assessments and regular QA visits Teaching and assisting with grocery shopping, meal preparation, nutrition, managing finances and budgeting, housekeeping and personal hygiene Social skills development and training Guidance in the identification of leisure and vocational activities Teaching the use of public transportation Guidance and teaching of the appropriate management of safety and health Independent living skills guidance and instruction Anger management skills Coping skills Behavioral modification techniques Crisis intervention Contact and collaboration with referral sources and other professionals Contact and intervention regarding employment as appropriate Accessing and coordination of community resources Linkage of client to other necessary service providers as appropriate Development of the service plan, progress reports/notes, and discharge summary The Quality Assurance Specialist duties will consist of but not limited to the following: Assist direct support professionals with maintaining regular contact with their clients, supervisor, and other health professionals within the community Ensure direct support professionals obtain and submit documentation of their client's medications as well as all other related documentation related to their health and safety. Review supervision binders biweekly to ensure each direct support professional has fully completed the supervision form and signed the document for each client. Provide training and reinforcement of activities and related interventions with direct support professionals at orientation, annually and on an as needed basis. Assist direct support professionals with linkage to all other supportive services for their clients (substance abuse evaluation/treatment, outpatient therapy, medication management and any other service appropriate for mental health, physical health and safety). Maintain complete confidentiality at all times with all staff; ensuring all client information remains confidential. Ensure direct support professionals sign notes on a weekly basis as well as signed ISP's, quarterlies, discharge summaries and all other corresponding documentation. Audit client files on a biweekly basis to ensure compliance and update electronic database to include all client information. Enforce direct support professional submission of safety plans for each client as well as review and updating as needed. Review direct support professional daily notes on a weekly basis to ensure goal progress or lack of is documented (weekly note or non-billable note). Send revision requests within 48 hours of receiving documentation Enforce weekly Sunday deadline for progress notes. Review individualized service plans and quarterlies of direct support professionals; send revision requests within 48 hours of receiving documentation. Identify coverage when cancellations or interruptions to a client's Plan for Supports/Schedule occurs Provide necessary coverage/support in accordance with a client's Plan for Supports/Schedule when unable to identify an alternate DSP to provide mandatory coverage Assist DSP's with individualized direct instruction within the client's home environment on client's individualized goals/needs as instructed by CEO or Program/Clinical Director Conduct employee education/training on client documentation completion (ISP's and quarterlies) Retrieve outstanding documentation for client files on a daily basis; contact case managers or DSP's for outstanding staff (i.e. SIS Assessments or quarterlies). Maintain spreadsheet database consisting of each client and mandatory client file documentation dates and outstanding information for weekly auditing purposes. Assist with the facilitation of monthly training for employees (staff meetings and other trainings as deemed appropriate and necessary). File all client chart documentation daily. Purge and discharge client files, filing them into the correct location. Compose and construct new client admission files with necessary documentation. Compose and construct necessary informational binders for client homes. Facilitate monthly supervision with each DSP and file corresponding documentation in supervision binder File attendance logs in designated binder weekly. Perform after-hours and weekend on-call for staffing and emergency client needs. Perform regular quality assurance visits/calls, documenting reported information. Assist and support human resources with answering office phones and greeting guests as needed. Other duties as assigned.

$20.00 - $25.00 per hour
