We're seeking to hire Prevention Education Case Managers. Case manager will provide educational support to assist with student development. Primary responsibility is ensuring and maintaining 85-100% student attendance. Prevention practices are implemented to attract parental participation to meet CPS attendance requirements. The secondary responsibility of the Case Manager is to conduct home visits. Students with below average attendance and chronically truant need assistance with removing barriers that impede the stabilization of their attendance.
Responsibilities:
conduct daily homevisits Record data for all students. Develop relationships and alliances with school staff, family members and community providers. Ensure that all daily home and school visits are performed and documented. Submit daily reports Participate in all teaming conferences needed to support students and family. Perform other duties as assigned
Qualifications :
High school or equivalent Ability to pass Background check Knowledge of Chicago community areas Verbal and written communication skills Strong computer skills Must have transportation, insurance and drivers license Bi-lingual candidates are needed to work in various community areas
