Position Overview Vivery is seeking a Community Partnership Specialist to lead the implementation, adoption, and ongoing success of Vivery programs across the Chicago region. This role will work closely with food pantries, health systems, Federally Qualified Health Centers (FQHCs), and community-based organizations to integrate Vivery into partner workflows, support frontline staff, and ensure programs achieve meaningful community impact. The ideal candidate is passionate about health equity, enjoys working directly with community members and partner organizations, and can effectively balance relationship management, program coordination, training, and community engagement. This role requires regular attendance at community and pantry events, where the Community Partnership Specialist will directly assist neighbors with enrolling in and utilizing Vivery services, while serving as a trusted ambassador for the organization. Key Responsibilities Partner Implementation & Program Management Lead implementation and onboarding efforts with community and healthcare partners across the Chicago area. Work with food pantries, health systems, FQHCs, and community-based organizations to integrate Vivery into existing workflows. Develop and manage implementation plans, timelines, milestones, and success metrics. Monitor program performance and proactively address operational challenges. Coordinate across internal and external stakeholders to ensure successful program launches and ongoing partner success. Relationship & Account Management Serve as the primary point of contact for assigned community and provider partners. Build and maintain strong relationships with leadership and frontline staff. Conduct regular check-ins and partner success meetings. Support partners in maximizing Vivery adoption and impact. Identify opportunities to improve workflows, engagement, and program outcomes. Community Engagement & Neighbor Support Represent Vivery at community events, health fairs, food distributions, pantry events, and partner gatherings throughout Chicagoland. Directly assist neighbors with account creation, enrollment, and onboarding onto the Vivery platform. Educate community members about available resources and the value of Vivery services. Serve as a trusted, culturally responsive resource for community members seeking food and health-related support. Collect community feedback and share insights with internal teams to improve programs and user experience. Provider Training & Adoption Deliver onboarding and training sessions for healthcare providers, care managers, community health workers, social workers, and other partner staff. Create and maintain training materials, workflow guides, and best practices. Support provider and staff adoption of Vivery tools and processes. Provide ongoing support and troubleshooting to partner organizations. Promote consistent utilization of Vivery across partner sites. Reporting & Continuous Improvement Maintain accurate records of partner engagement, implementation activities, and program outcomes. Track key performance metrics related to program adoption and partner success. Prepare reports and updates for leadership and external stakeholders. Identify opportunities for process improvement and enhance community impact. Qualifications Bachelor's degree in Public Health, Community Health, Social Work, Healthcare Administration, Human Services, or a related field with 2-3 years of relevant professional experience, OR Master's degree in Public Health (MPH), Social Work (MSW), Healthcare Administration (MHA), Community Health, or a related field. Relevant internships, fellowships, practicum experience, and community-based experience will be considered. Strong interpersonal and relationship-building skills. Excellent written and verbal communication skills. Strong organizational and project management abilities. Commitment to advancing health equity and addressing food insecurity. Reliable transportation and ability to travel regularly throughout the Chicago metropolitan area. Ability to work occasional evening and weekend hours to support community engagement activities and events. Preferred Qualifications Bilingual in English and Spanish. Experience working with food pantries, healthcare organizations, FQHCs, social service agencies, or community-based organizations. Experience with Food as Medicine, Social Determinants of Health (SDOH), population health, or community health initiatives. Experience facilitating training, workshops, or community outreach programs. Familiarity with the Chicago healthcare and community services landscape. What Success Looks Like Community and provider partners successfully incorporate Vivery into their daily operations. Neighbors are effectively onboarded and connected to resources through Vivery. Provider staff are trained, supported, and actively utilizing Vivery workflows. Partner programs remain on track and achieve adoption and engagement goals. Vivery is recognized as a trusted and visible presence within the Chicago community. Compensation & Benefits $65,000-$75,000 annually, commensurate with experience.
