What You'll Do Develop and grow referral relationships with physicians, hospitals, skilled nursing facilities, home care providers, and community agencies within the assigned territory Serve as a trusted liaison between Andwell and referral partners, providing information on home health, hospice, and palliative care services Create and execute territory development plans focused on increasing awareness, strengthening partnerships, and growing appropriate referrals Conduct regular sales calls, outreach visits, and presentations to referral sources to promote Andwell services Educate healthcare providers, patients, and families on available services, eligibility criteria, and care pathways Support the referral-to-admission process by facilitating communication, gathering required information, and coordinating with admissions and clinical teams Build and maintain strong professional relationships with physicians, discharge planners, social workers, facility staff, and community partners Track referral activity, maintain accurate CRM documentation, and monitor trends and opportunities within the assigned territory Represent Andwell at community events, educational programs, and outreach initiatives to increase visibility and engagement Maintain current knowledge of Andwell programs, service offerings, regulatory requirements, and market trends Uphold patient confidentiality and comply with all regulatory and organizational standards What You'll Bring Bachelor's degree in healthcare administration, business, marketing, or a related field preferred Three (3) or more years of experience in healthcare sales, provider relations, business development, or referral management Experience working with physicians, hospitals, skilled nursing facilities, or other healthcare referral sources Strong relationship-building and consultative sales skills with the ability to develop and maintain long-term partnerships Proven ability to generate leads, grow referral pipelines, and meet territory growth goals Excellent communication and presentation skills with the ability to effectively represent services to healthcare partners Strong organizational and territory management skills with the ability to work independently Proficiency with Microsoft Office and Customer Relationship Management (CRM) platforms Analytical mindset with the ability to track referral trends and identify opportunities for growth Integrity, adaptability, and a collaborative, team-oriented approach Commitment to mission-driven, patient-centered care Be part of a collaborative, mission-driven team where clinical expertise, compassion, and relationships truly matter.
