About MedZed MedZed is a leader in delivering value-based, technology-enabled social support to high-cost Medicaid members who are unreachable and disconnected from their primary care provider. We combine innovative outreach and care models with technology to find and engage members, connect them to primary care, and improve outcomes. Our intervention yields reduced costs and assists our members in enhancing their quality of life. Position Overview Under the supervision of the Director of Community Impact Partnership Programs, The Community Engagement Ambassador (CEA) is a California-based, regional role responsible for executing field-based outreach and business development activities to support referral generation into MedZed's CalAIM Enhanced Care Management (ECM) and Community Supports (CS) programs. This position requires a consistent field presence and support assigned regions by developing and maintaining relationships with external organizations, increasing program awareness, and facilitating referral activity in alignment with established processes and performance expectations. Key Responsibilities Business Development & Outreach Execution  Develop and maintain working relationships with community-based organizations (CBOs), health care providers, housing/homelessness services, behavioral health agencies, and county/municipal entities.  Execute outreach activities such as tabling events, presentations, and partner training in support of referral growth goals established by the Director of Community Impact Partnership Programs.  Meet established activity and documentation requirements related to outreach volume, partner engagement, and referral support.  Maintain consistent partner follow-up to support engagement and referral continuity.  Serve as a field point of contact for assigned partners, escalating issues as appropriate. Regional Coverage & Field Operations  Support community engagement activities across an assigned California region or group of counties.  Maintain a regular field presence 3-4 days per week, including partner meetings, site visits, and outreach events.  Travel within California as required; occasional overnight or flight travel may be necessary based on regional coverage.  Represent MedZed professionally at external meetings and events. Program Knowledge & Referral Support  Develop expert knowledge of California's CalAIM Enhanced Care Management (ECM) and Community Supports (CS) programs.  Understand and communicate Medi-Cal eligibility requirements and referral criteria.  Support referral generation by ensuring partners understand referral submission requirements.  Coordinate with internal intake and operations teams to support timely referral handoff and follow-up. Systems, Documentation & Compliance  Document outreach activity, partner interactions, and referral-related information in Salesforce or designated systems.  Adhere to established workflows, SOPs, and documentation standards.  Support data accuracy and reporting requirements related to engagement and referral activity. Qualifications  Must be in California and able to travel regionally.  1-2 years of experience in outreach, sales, healthcare services, account coordination, or a related field.  Strong professional communication and relationship management skills.  Ability to work independently in a field-based environment.  Familiarity with CalAIM and Medi-Cal policies or willingness to learn.  Maintaining an active driver's license and vehicle Insurance.  Proficiency with Microsoft Excel, Power Bi, Salesforce is a plus. Education  Bachelor's degree in business, Public Health, Health Administration, Communications, Sociology, or a related field preferred but not required.  Equivalent lived or work experience may be considered in absence of degree. Position Requirements Ability to travel locally and regionally within California. Ability to work in a field-based role with limited direct supervision. Ability to manage multiple priorities and maintain accurate documentation. Physical Requirements This position requires regular travel throughout California which may involve driving, walking, standing by automobile or airplane and navigating indoor and outdoor environments, including stairs and uneven surfaces. The employee must be able to constantly operate a computer, keyboard, and other office productivity machinery, such as a calculator, copy machine, and computer printer. The person in this position may need to occasionally move about inside the office/remote workspace to access file cabinets, office machinery, or other resources. The employee may occasionally be required to lift and move objects weighing up to 10 pounds (e.g., a laptop, files, or small office equipment). Clarity of vision at approximately 20 inches or less is required for working with computers and reading small print.
