Home Care Marketing (Community Liaison) Blize Healthcare - 2.8 Hercules, CA Job Details Full-time $20 - $30 an hour 2 hours ago Benefits Fuel reimbursement Dental insurance Paid time off Vision insurance Qualifications Customer relationship building Client rapport building Presentations for conferences or internal meetings Full Job Description Blize Healthcare is seeking a compassionate and driven Home Care (Private Duty) Community Liaison to join our Team. In this role, you will build and maintain relationships with referral sources. Including physicians, hospitals, skilled nursing facilities, and community organizations, to support patient access to home care sources.
Responsibilities:
Develop and maintain strong relationships with healthcare providers and community partners. identify and secure new referral sources through education and outreach. Represent Blize Healthcare at trade show, community events and professional gatherings. Monitor and analyze referral and admissions data to identify trends and opportunities. Negotiates per diem or per visit payments rates for creative bunding of home care services. Collaborates with the hospital's discharge planning personnel, utilization review department and Insurance Case Managers to facilitate safe early discharge. interview the patient, family, and caregiver and discuss the home situation, current needs, and any psychosocial factors that are relevant to the care plan.
Requirements:
Strong communications, interpersonal, and presentation skills. Proven ability to build and maintain professional relationships. Sales or marketing experience, preferably in healthcare. Associate's degree or equivalent experience. Ability to lift up to 50 LBS as needed. Reliable transportation for travel with assigned territory.
