Blize Healthcare. Blize Healthcare is seeking a compassionate and driven Home Care Community Liaison to join our team. In this role, you will build and maintain relationships with referral sources, including physicians, hospitals, skilled nursing facilities, and community organization.
Responsibilities:
Develop and maintain strong relationships with healthcare providers and community partners. Identify and secure new referral sources through education and outreach. Represent Blize Healthcare at trade shows, community events, and professional gatherings. Educate the community about home Care services. Monitor and analyze referral and admissions data to identify trends and opportunities. Collaborate with clinical teams to ensure seamless patient transitions to Private Home Care.
Requirements:
Strong communication, interpersonal, and presentation skills. Proven ability to build and maintain professional relationships. Sales or marketing experience, preferably in healthcare. Knowledge of Home Care continuum, services, and regulatory requirements. Associate's degree or equivalent experience. Ability to lift up to 50 pounds as needed. Reliable transportation for travel within assigned territory.
