About EliaCore EliaCore partners with physicians, medical providers, senior care communities & hospitals to improve patient outcomes through proactive, Medicare-supported care management programs. We believe the best healthcare begins long before a hospital admission. Through collaboration, education, and ongoing support, we work alongside physicians and community partners to deliver meaningful clinical solutions that improve quality of life. Overview The Physician & Community Liaison serves as EliaCore's primary relationship representative throughout the community. This individual develops trusted partnerships with physician practices, assisted living communities, independent living communities, skilled nursing facilities, hospitals, and other healthcare organizations. The successful candidate will focus on education, relationship development, community engagement, and helping healthcare partners understand how EliaCore's programs can enhance patient care while reducing administrative burden for providers. Physician Relation Responsibilities Build and maintain relationships with primary care physicians, specialists, and healthcare providers. Schedule and conduct educational visits with physician practices. Introduce providers to EliaCore's care management programs and available clinical resources. Coordinate physician onboarding and implementation. Serve as a trusted point of contact for participating providers. Maintain ongoing communication to ensure provider satisfaction and engagement. Community Relations Responsibilities Build relationships with Executive Directors, Wellness Directors, Admissions Teams, and Clinical Leaders throughout senior living communities. Conduct routine community visits. Coordinate educational presentations and Lunch & Learns. Support communities in understanding available resources that benefit residents and providers. Foster long-term partnerships through consistent communication and exceptional service. Partnership Development Identify opportunities to establish new physician and community relationships. Coordinate introductions between prospective partners and EliaCore leadership. Support the successful onboarding of new physician practices and community partners. Maintain accurate records of outreach activities and follow-up communications. Experience Proven experience in business development with a strong background in healthcare environments. Demonstrated success in client management, and territory expansion within healthcare settings. Strong negotiation skills with the ability to close deals effectively while maintaining positive relationships. Excellent communication skills for engaging diverse audiences including physicians, hospital staff, and community organizations. Experience in outside sales roles involving product demos, enterprise selling strategies, and meeting or exceeding sales goals. Organizational skills necessary for managing multiple relationships, scheduling meetings, and maintaining detailed records. Familiarity with Google Suite products for documentation and PowerPoint for presentations; analysis skills are highly valued. Pay:
$60,000.00 - $100,000.00 per year
