The Community Liaison educates our community about our services and programs, builds relationships with accounts for home health referrals and generates revenue, and maintains a positive presence in the community.

Qualifications:

2-3 years of Home Health experience Excellent customer service skills Excellent verbal and written communication skills Drive to succeed

Responsibilities:

Maintain a current log of all referrals, admissions, nonadmits hospitalized patients, and patient insurance. A copy of log will be submitted to the office for payroll and administrative purposes. Identify 35 new sources to educate on Hygeia's services and specialty programs each month. Identify potential growth areas and open new accounts.
