You'll be a trusted resource, an advocate, and a guide for families navigating some of life's most challenging transitions. Your work helps seniors and adults remain in the place they love most—their home—surrounded by their memories, routines, pets, neighbors, and the life they've built. What You'll Do Build meaningful relationships with hospitals, rehabilitation centers, skilled nursing facilities, assisted living communities, physicians, and other healthcare partners Educate referral sources about the benefits of quality in-home care Meet with families to understand their needs and help them create a plan that supports independence and safety at home Work closely with our nursing and operations teams to ensure seamless care transitions Serve as a trusted resource for families during times of uncertainty Develop and grow your territory through relationship-building and community engagement Why This Role Matters Every referral represents a family facing an important decision about a loved one's future. We're looking for a compassionate, relationship-driven professional who enjoys helping others, building trust, and connecting families with the care they need. Sales, healthcare, senior living, or home care experience is preferred, along with strong communication skills, self-motivation, and reliable transportation.
