BridgeCare Medicine | South Jersey & Philadelphia $30-$40/hour, depending on relevant experience | Approximately 10-15 hours/week | Flexible scheduling BridgeCare Medicine is a physician-led practice focused on care in skilled nursing and post-acute settings. We are seeking a personable, organized community liaison to help build relationships with nursing homes, rehabilitation centers, and other healthcare organizations across South Jersey and the Philadelphia area. You will work directly with our physician founder to identify opportunities, connect with facility leaders, and coordinate introductory meetings. What you will do Build relationships with nursing-home administrators, directors of nursing, social workers, and other healthcare leaders. Identify facilities interested in discussing physician coverage and medical director services. Conduct local outreach through calls, emails, facility visits, and networking events. Arrange introductory meetings with our physician founder and follow up on conversations. Maintain a simple tracker of contacts, outreach activity, and next steps. Who would be a great fit Backgrounds in social work, case management, nursing-home admissions, discharge planning, nursing, rehabilitation, or healthcare business development are welcome. Experience in skilled nursing, long-term care, rehabilitation, or long-term acute care hospitals (LTACHs). Familiarity with South Jersey and Philadelphia healthcare organizations; existing professional relationships are strongly preferred. Comfort initiating conversations, organizing your own outreach, and following through independently. Strong communication and organizational skills, with the ability to attend locally arranged meetings. Schedule and compensation Flexible part-time work combining remote coordination with local outreach and facility visits. You can organize your outreach around your own schedule, coordinating meetings with facility availability. Expected workload is approximately 10-15 hours per week. Pay is $30-$40 per hour based on relevant experience. Approved local travel expenses are reimbursed. Employer-sponsored health insurance is not offered. Responsibilities do not include counseling, clinical social-work services, or patient care. A clinical or social-work license is not required.
