We are seeking a dynamic and compassionate Community Liaison Assistant to serve as a vital bridge between community members and our organization. In this role, you will foster strong relationships, facilitate effective communication, and support community engagement initiatives. Your enthusiasm and leadership will help create a welcoming environment where diverse voices are heard and community needs are addressed proactively. This paid position offers an exciting opportunity to make a meaningful impact through collaboration, outreach, and program development. Responsibilities Build and maintain positive relationships with the County Offices of Assistance. Coordinate with consumers who are applying for Medicaid. Collect paperwork and prepare applications. Schedule assessments for caseworkers with the Philadelphia Corporation for Aging. Schedule assessments for the Managed Care Organizations(MCOs). Home visits to pick up paperwork like bank statements, identification cards, etc. Skills Proven experience coordinating with the CAOs, PCA and MCOs. Strong organization skills. Demonstrated ability to work with others. Competence in data collection and follow up for program evaluation and reporting purposes .Excellent communication skills. Background in public health, social work, home care, case management or related fields is highly desirable. Leadership qualities with the ability to motivate others and foster teamwork. Excellent administrative skills with attention to detail in record keeping and scheduling.
