• We are currently seeking a Home Health Sales and Marketing Liaison to work in the Kern County area specifically in Delano, Wasco, Shafter, Arvin, Lamont, Tehachapi and Taft.

Overview:

The Home Health Marketing Rep will have existing relationships with patient Home Health referral sources and Home Health Community and Institutional contacts.

Home Health Sales and Marketing Job Expectations and Responsibilities:

• Assists in the coordination of services for patients discharged from hospitals and other referral sources into the home health program
• Responsible for obtaining valid Medicare and other referrals
• Assists in obtaining doctors signatures for various home health care documents
• Assists in coordination of home care needs with the agency in accordance with the physician's plan of care.
• Assists in the resolution of issues between families, referral sources, patients and the agency
• Works closely with agency personnel to identify community resources
• Participates in case conferences with supervisory and field personnel.
• Attends meetings, participating in both external and internal committees and organizations
• Represents the agency at all times in a professional, ethical and loyal manner
• Complies with agency policies and procedures

Minimum Qualifications:

• Candidate should have experience in Marketing/Sales in Home Health referrals from physicians, hospitals, rehabs, and assisted living communities
• Must have a CA Driver's License with an automobile that is insured in accordance with state and/or Agency requirements and is in good working order
• Have satisfactory references from previous (or current) employers and/or professional peers.
• Have excellent interpersonal and telephone skills
• Have excellent time management and organizational skills
• Have excellent ability to communicate verbally and in writing
• Ability to work independently
• Bachelor degree preferred but not required
• Customer Relationship Management (CRM) experience a plus

From $21.00 per hour
