Shift:
Full-Time | Monday-Friday | 8:00 AM•4:00 PM Includes 30-minute unpaid lunch Join Our QPCS Team! Quality Patient Centered Services (QPCS) is seeking a dedicated and compassionate MAT Program Specialist•Advanced to support individuals within the Pennsylvania Department of Corrections system. This role focuses on Medication-Assisted Treatment (MAT), case management, counseling, reentry coordination, and treatment planning for incarcerated and reentering individuals. Position Summary The MAT Program Specialist•Advanced provides direct support, counseling, care coordination, and treatment planning services to individuals participating in Medication-Assisted Treatment programs. The specialist collaborates with SCI staff, parole staff, treatment providers, and community resources to ensure continuity of care and successful reentry outcomes. Key Responsibilities Coordinate MAT referrals and appointments for medication and treatment services Conduct assessments and provide counseling using various treatment modalities Assist with reentry planning and coordination of community-based services Track and maintain data related to MAT participants using spreadsheets/databases Complete required documentation in systems such as Captor and Sapphire Conduct GPRA assessments (initial, 6-month, and discharge) Meet with individuals in correctional facilities, parole offices, treatment programs, and community settings Participate in weekly Vivitrol conference calls and required meetings/trainings Collaborate with multidisciplinary treatment teams and DOC staff Maintain compliance with
HIPAA, DOC
policies, and confidentiality standards
Qualifications Required:
Bachelor's Degree in Social Work, Human Services, or related field Experience in case management, counseling, behavioral health, or substance use treatment Strong communication and organizational skills Proficiency in Microsoft Excel and documentation systems Ability to work collaboratively with multidisciplinary teams
Preferred Skills:
Knowledge of social work principles, counseling techniques, and casework practices Understanding of substance use disorders, MAT programs, and reentry services Familiarity with HIPAA and confidentiality regulations Experience working within correctional or community behavioral health settings Why Join QPCS?
