reviews Full-time Overview We are seeking a dedicated and detail-oriented MDS (Minimum Data Set) Coordinator to join our team. In this vital role, you will oversee the comprehensive assessment and documentation of patient care in our skilled nursing facility, ensuring compliance with federal and state regulations. Your expertise will facilitate accurate coding and support quality care delivery. This position offers an exciting opportunity to impact patient outcomes through meticulous clinical documentation and interdisciplinary collaboration. Duties Lead the completion and accuracy of the MDS assessments for each resident, ensuring all data reflects current clinical status and care needs. Collaborate with nursing staff, Therapy, social worker, and other interdisciplinary team members to gather comprehensive patient information. Ensure compliance with all applicable regulations including CMS (Centers for Medicare & Medicaid Services) standards. Utilize EMR (Electronic Medical Record) system (PCC) to document assessments, review medical records, and update resident information efficiently. Perform thorough documentation reviews to support proper coding using
ICD-10 CPT
codes to ensure accurate reimbursement. Conduct utilization review and management activities to optimize patient care plans while maintaining regulatory compliance. Maintain up-to-date knowledge of Medicare regulations, managed care policies to ensure ongoing compliance. Experience Proven experience in clinical documentation improvement within a skilled nursing facility setting. Strong background in MDS coordination with familiarity in completing assessments aligned with CMS requirements. Knowledge of ICD-10 coding systems, CPT coding. Experience working with EMR/EHR system (PCC). Familiarity with Medicare regulations, managed care processes, discharge planning Excellent understanding of medical terminology, anatomy, physiology, and medical records management. Strong organizational skills with attention to detail; capable of managing multiple assessments simultaneously while adhering to deadlines.
