In this vital role, you will oversee the comprehensive assessment and documentation of patient care needs within skilled nursing facilities, hospitals, or outpatient settings. Your expertise will ensure accurate completion of Minimum Data Set (MDS) assessments, facilitating optimal care planning, regulatory compliance, and reimbursement processes. The ideal candidate will possess strong clinical knowledge, exceptional organizational skills, and a passion for improving patient outcomes through meticulous documentation and case management. Responsibilities Conduct thorough MDS assessments in accordance with federal and state regulations, including Medicare and Medicaid requirements. Collaborate with interdisciplinary teams to gather clinical information, ensuring accurate reflection of patient status and needs. Review medical records, clinical documentation, and coding details such as CPT (Current Procedural Terminology), ICD-9/10 (International Classification of Diseases), DRG (Diagnosis-Related Group), and other health information to support precise assessment completion. Manage utilization review and utilization management processes to optimize patient care while maintaining compliance with NCQA standards. Utilize electronic health record (EHR) systems such as Epic, Cerner, Athenahealth, or eClinicalWorks to document assessments and update patient records efficiently. Facilitate discharge planning, hospice care coordination, and transition of care activities for inpatient, outpatient, or home care settings. Ensure adherence to HIPAA regulations by maintaining confidentiality and security of medical records and documentation. Requirements Proven experience with MDS coordination in long-term care facilities, hospitals (Level I or Level II trauma centers), or outpatient clinics. Strong knowledge of clinical documentation improvement strategies, medical terminology, anatomy, physiology, and medical coding includingICD-10/CPT
coding systems. Familiarity with EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks is essential. Experience with managed care programs like Medicare and Medicaid is highly preferred. Critical care experience or ICU background enhances understanding of acute patient needs; nursing or nursing home experience is a plus. Knowledge of healthcare regulations including HIPAA standards and NCQA accreditation requirements. Excellent organizational skills with the ability to review complex medical records efficiently while maintaining attention to detail. Strong communication skills for collaborating with multidisciplinary teams across inpatient, outpatient, hospice, or home health settings.
