1 yr of SW experience required Hospice experience preferred Must be willing to travel in all weather conditions Strong computer and technology skills Excellent organizational and communication skills Ability to work independently and within a multidisciplinary team Active patient contact within past three years, preferred.
SW Qualifications:
Must meet at least one of the following: (a) Have an MSW degree from a school of social work accredited by the Council on Social Work Education (CSWE) and one year experience in a health care setting; (b) have a baccalaureate degree in social work (BSAW) from a school of social work accredited by the CSWE and one year of experience in a health care setting and be supervised by a MSW accredited by the CSWE; or (c) have a baccalaureate degree in psychology, sociology or other field related to social work and at least one year of social work expierience in a healch care setting and be supervised by a MSW accredited by the
CSWE. SW
Skills:
have good oral and written communication skills; be of excellent health and possess a high energy level; demonstrate a commitment to excellent patient care and customer service.
Knowledge/Skills/Ability:
Ability to work independently, make accurate, and at times, quick judgments. Ability to supervise others appropriately. Ability to respond appropriately to crisis outside of a hospital setting. Acceptance of and adaptability to different social, racial, cultural and religious modes. Completes Hospice training program.
Essential Functions:
Regularly assesses and reassesses the nursing needs of the Hospice patient. Provides Hospice social work services and treatments. Initiates social work procedures appropriate for the patient's Hospice care and safety. Observes signs and symptoms and reports to the physician and IDG members any unexpected changes in the patient's physical or emotional condition. Teaches, supervises and counsels the Hospice patient and family members about providing care for the patient. Develops and re-evaluates the patient/family care plan in conjunction with IDG to meet needs and maintain continuity of care. Performs specific social work procedures as needed (e.g., treatments, management of symptoms) following doctor's orders. Maintains records as required by Hospice. Coordinates the implementation of the plan of care for patients residing in SNF, NF, ICF or MR.
