Job Description
Under the direction and supervision of the Clinical Supervisor and Program Director, this position provides an array of training, support and activities of daily living services to individuals diagnosed with severe mental illness in the community setting. The position will deliver direct-care services (up to 15 hrs. per client) to support clients with mental health problems within their home and community. Duties and Responsibilities Consistently assessing client's levels of risk, and carrying out their needs based on initial assessment based on Mental Health Skill Building 7 Core competencies. Facilitates care coordination with other service providers to enlist existing supports in determining and meeting service objectives and to identify additional resources and supports Prepares written individualized service plans and treatment update Summaries-Quarterly and/or Monthly, incorporating input from clients, other service providers, wraparound team members to identify problem areas and needs, service objectives, and intervention strategies Planning, recommending and coordinating services that improve the level of independence and stability in the community to prevent substance abuse and to combat social problems. Responds to crisis situations on a twenty-four hour a day availability Completes progress reports in reference to service delivery within 24 hours Collaborating with physicians, therapist, psychiatrist, case manager, substance abuse provider to develop, plan and manage treatment. Referring client to other agencies for treatment/services to help in recovery from physical or mental illness. Monitoring, evaluating, and recording client progress with that of treatment goals. Participate in case supervision and training offered by agency Skills and Specifications Knowledge of mental health laws and regulations. Ability to maintain friendly public relations. Ability to deal with stress and effectively manage time. Ability to take initiatives, advocate for and decisions for the betterment of the clients. Should have strong writing and critical thinking skills. Strong Computer skills Education and Qualifications A Bachelor's Degree in Social Work, Psychology, Counseling, or a related field from an accredited college or university with a minimum one year of experience in the mental health field. Must meet the criteria to be a Qualified Mental Health Professional (QMHP A or T).
