Position Summary Under the supervision of a Physician, provides to individuals and groups nursing care requiring the application of basic knowledge of the biological, physical, behavioral, social and nursing sciences. Job requires knowledge; skill and ability to provide care for neo-natal, pediatric, adolescent, adult or geriatric patient population.
Daily Tasks:
Assist Director with educating and coaching counseling staff Counsel employees and implement appropriate disciplinary action, when necessary Work with Director and other leadership staff to Interview, hire, and orient new employees Provide direction and assistance to PCAs regarding the SUD program Take part in pulling and evaluating data for Chemical Dependency Dashboard Worksheet Complete chart audits, as indicated per TJC, OhioDBH, etc. Review and evaluate outpatient SUD occurrence reports Conduct and document brief individual counseling sessions and all therapeutic activities Facilitate group therapy Attend Treatment Team, Monday through Friday Review and assign goal work Answer phone calls Complete discharge summaries Communicate with the Registration Specialist regarding the Treatment Team's recommendation of Level of Care Family consultations as needed Case management activities Education lectures Formulate Master Treatment Plan for patient and review progress Read patients goal work and provide suggestions for further learning
Weekly Tasks:
Individual weekly review to assess patient progress
As Needed or Requested:
Urgent family sessions/interventions for noncompliant or resistant patients with their counselor(s) and addictionologist Supervise Aftercare groups and write Aftercare letters regarding progress as needed Help patients plan therapeutic leaves Monitor all patients' AA attendance
Monitor Sober Living Residents Monthly Tasks:
Chart review quarterly Facilitate counselor meetings quarterly Attend manger meetings monthly Attend Shepherd Hill Cabinet meetings quarterly Weekend on-call Ensure patient satisfaction surveys are completed and turned in to the
Director Annual Tasks:
Attend workshops for further education and keep licensure current Complete WebEd courses and other required education (i.e., patient safety, Aegis, etc.) Maintain CPR certification Meet with counselors & PCAs for annual reviews LMH is accredited by DNV and TJC, and as such, may require specific annual education related to specialty certifications and standards.
