Job Description
General Description:
A Community Navigator at Tewa Roots Society provides trauma-informed, land-based, and culturally rooted support primarily to clients in Nanbe' Owingeh and surrounding Pueblos. This role fosters relationships, collaborates across programs, actively engages in community, supports clinical care, and engages with clients both indoors and outdoors. An ideal candidate has extensive knowledge of resources that support mental health and overall wellness that is tailored to the needs of Indigenous individuals and their families.Position Responsibilities:
Conduct thorough Comprehensive Community Support Service assessments to identify client needs, functional impairments, strengths, and barriers Develop individualized treatment/care plans with short-term and long-term goals, outlining steps for achievement Connect clients with culturally appropriate services, including healthcare, social services, and community resources Advocate for clients' needs and rights, ensuring they receive the necessary support and resources Track client progress through a strengths-based lens, adjust care plans as needed, and provide ongoing support Maintain accurate and detailed case records, including case notes, reports, and assessments Submit superbills for services provided Facilitate communication between clients, their families, and service providers Provide support and intervention during times of crisis (within scope of practice) Transport clients to and from appointments Work collaboratively with a multidisciplinary team to ensure holistic care Educate clients and their families about available resources and services Manage own schedule and maintain consistent and compassionate communication; be effective as a team member and in working independently Complete documentation in compliance with TRS standards, in an ethical and timely manner and maintain HIPAA compliance regarding PHI and collateral (i.e. art storage) Consistently engage in anti-oppressive and anti-racist self-work and actively seek out relevant professional development opportunities Establish positive and trusting relationships with clients, colleagues, and Pueblo community members; support/attend TRS and community events when appropriate Maintain confidentiality of all personal information of clients and community members, and seek supervision to support with navigating any dual relationships that may arise Maintain all required licenses and obtain the necessary certifications/training required for adventure and group services Adhere to all policies and procedures of the Pueblo of Nambe, TRS' Risk Management Curriculum, and Therapy Service's Program Manual, and upholdTRS' Norms and Expectations Agreement Note:
Staff must pass a drug screening at the onset of employment and may be subject to additional screening in response to Tribal leadership request, a work incident, or policy violation.Preferred Qualifications:
Experience living and/or working in a Tribal community Speaker or language learner of the Tewa language Active certification as a LADAC, CPSW, and/or CCSS Valid driver's license with a clean driving record First Aid, CPR, and/or other applicable certifications or the ability to obtain certifications tailored to the needs of Indigenous individuals and their families.
