This role focuses on medical readiness, ensuring Service Members (SMs) meet required health standards through care coordination, documentation management, and education on available healthcare resources. Key Responsibilities Case Management & Care Coordination Manage Service Member cases from initial identification through resolution or administrative separation Coordinate care and treatment plans with military and civilian medical facilities Track referrals and ensure consistent follow-up with assigned Service Members Submit medical documentation and profile-related concerns to appropriate providers Create initial temporary profile entries in the

system within required timelines Medical Readiness Support Support Periodic Health Assessments (PHA), Soldier Readiness Processing (SRP), and Soldier Readiness Certification (SRC) Attend PHA events as needed to assist with readiness efforts Review and monitor health assessments, labs, and documentation for accuracy Educate Service Members on healthcare resources, including TRICARE and VA benefits Documentation & Reporting Maintain accurate medical records within the eCase system Upload and manage documentation in
