This position involves working closely with patients, families, and healthcare providers to facilitate effective communication and streamline care processes. The ideal candidate will have a strong background in patient service and medical terminology in a youth caring residential setting. Starting Pay Rates LPN - $28.32/hr. RN - $33.52/hr. Duties Collaborate with treatment team members to develop and implement individualized care plans for patients. Conduct patient assessments to identify needs and coordinate appropriate services. Facilitate discharge planning to ensure smooth transitions to home or other care settings. Provide education and support to patients and families regarding treatment options and care processes. Maintain accurate documentation of patient interactions, assessments, and care plans in compliance with medical standards. Serve as a liaison between patients, families, and healthcare providers to enhance communication and resolve issues. Monitor patient progress and adjust care plans as necessary based on ongoing assessments. Stay informed about current medical terminology, practices, and regulations related to medical-surgical and ICU settings. Skills Strong understanding of patient service principles and practices. Proficiency in medical terminology related to infectious disease care, critical care, and discharge planning. Excellent communication skills for effective interaction with patients, families, and healthcare professionals. Ability to perform thorough patient assessments to determine needs for care coordination. Strong organizational skills with attention to detail for maintaining accurate records. Ability to work collaboratively within a multidisciplinary team environment. Compassionate approach towards patient care with a focus on enhancing the overall patient experience.
