The nursing case manager will work collaboratively as a member of the interdisciplinary team.
ESSENTIAL COMPETENCIES
- Monitor and assess chronic and acute/episodic health care conditions for individuals and develop and maintain current health care plans in collaboration with the individuals' multidisciplinary clinical team and program staff.
- Obtain up-to-date/current medical records for new referrals to assess health care needs and appropriateness of residential placement to the programs.
- Assist program staff and providers in maintaining complete, organized and accurate medical information as part of the confidential record.
- Assist staff and providers with scheduling and preparing for medical, dental and emergency appointments. Attend/accompany/transport individuals to appointments as needed which may require driving mini-van or wheelchair van providing transportation independently.
- Implement staff training programs, including but not limited to: Bloodborne Pathogens, Nutrition and topics specific to program sites and the unique medical conditions of their individuals to assist staff, providers, and individuals in understanding behaviors that promote good health, nutrition and personal hygiene and prevent illness such as ensuring appropriate infection control practices are followed.
- Provide on-call coverage as defined by policy for emergency and episodic conditions.
- Provide on-site supervision, medication administration, first aid and emergency care to individuals as needed.
- Assist in locating and coordinating medical resources (i.e., specialties).
- Maintains stakeholder (DHS, OLTL, CARF, etc.) compliance.
- Performs daily sick call and triages resident medical care.
- Monthly/Weekly/Daily vitals and physical head to toe assessments based on resident diagnosis and medical needs.
- Ensures med room policies and procedures are followed.
- Conduct monthly meetings with supervisors to discuss/review health concerns, medical appointment reviews.15
- Provide regular and as-needed consultation to program staff, health care providers and families.
- Attend in-service, required trainings and residential staff meetings as outlined by the Director.
- Cover nursing needs when necessary and on an emergency basis at any of the Action Recovery Residential and Day Program.
- Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards, and safety standards. Complies with governmental and accreditation regulations.
- Ability to learn and apply computer systems in order to manage patient information.
- Oversee all aspects of medication within Day Program. Advocates for policies and services that promote the quality of life for individuals with disabilities and participates in activities that will positively influence the community's awareness of disabilities and applies nursing research to clinical practice and participates in nursing research studies.
- Participates in meetings and on committees and represents the department and hospital in community outreach efforts.
- Participates in multi-disciplinary quality and service improvement teams.
- Performs other duties as assigned.
ADDITIONAL DUTIES AND RESPONSIBILITIES
Completes all mandatory training sessions at orientation and annually thereafter, achieving a minimum of 12 hours annually, including required training related to brain injury. Attends scheduled staff meetings and all other meetings, as assigned. Maintains confidentiality of information related to the individuals served. Interacts in a respectful manner with individuals served, coworkers and visitors. Abides by all New Vitae Policies and Procedures. QUALIFICATIONS
Education:
Required:
Current, valid Licensure as a Practical Nurse (LPN) in the Commonwealth of Pennsylvania. Valid and clean PA Driver's License.Experience:
- At least two years of experience in a supervisory capacity with the developmentally disabled and/or TBI population is required.
WORKING CONDITIONS
Physical Demands:
o Able to sit, stand, and walk for periods of time o Able to hear ordinary conversation and phone communications o Fluent in English both verbally and in writing o Ability to lift a minimum of 50lbs.Equipment Operation:
o Able to operate common office equipment, including a computer o Has a valid driver's license and follows legal requirements when transporting resident(s)Hazards of Position:
