Job Description

We are seeking a dedicated and compassionate Medicine/Palliative Inpatient Physician Assistant or Nurse Practitioner to join our dynamic healthcare team. In this vital role, you will collaborate closely with physicians, nurses, and interdisciplinary staff to deliver comprehensive medicine and palliative care to patients with serious illnesses. Your expertise will help improve quality of life, manage complex symptoms, and support patients and their families through challenging health journeys. Pay:
$145,000.00 per year Job description: Practice Details 60-bedLong-term acute care (LTAC) stand alone facility Physician and APPs on daily APPs cross trained in both Medicine and Palliative Care to cover a multitude of clinical needs for our patient population Strong specialist support
APP Responsibilities:
  • Conduct thorough patient assessments, including vital signs, physical examinations, and symptom evaluations to inform personalized care plans.
  • Manage acute pain and symptom control using evidence-based interventions
  • Collaborate with multidisciplinary teams to develop and implement comprehensive palliative care strategies aligned with patient goals and clinical guidelines.
  • Utilize Epic as our electronic health record (EHR) system.
  • Assist in admissions, discharge planning, Rapid Responses and Code Management, ensuring seamless transitions to the next level of care while coordinating with case managers and family members.
  • Maintain compliance with HIPAA regulations and hospital policies while delivering safe, aseptic techniques.
  • Expected census of up to 15 patients daily. Experience
  • Proven hospital experience in inpatient settings such as ICU, Level I or Level II trauma centers, or emergency rooms with exposure to critical care environments.
  • Demonstrated proficiency with EMR systems including Epic.
  • Background in acute care specialties such as internal medicine, hospital medicine, emergency medicine, or geriatrics is highly desirable.
  • Familiarity with ventilator management, airway management techniques, dialysis procedures, and infectious disease care enhances your ability to provide comprehensive inpatient support. However, this responsibility is that of the Pulmonary Team and Respiratory Therapists.
  • Prior involvement in Palliative Care practices is a plus.
