  • Full-time Conducting thorough medical evaluations for elderly, frail, chronically ill, and homebound patients, focusing on comfort and quality of life.
Managing Electronic Medical Record (EMR) documentation, ensuring timely completion within 72 hours of patient visits. Developing and regularly reviewing individualized care plans with patients, families, and caregivers, focusing on symptom management and quality-of-life improvements. Assessing and managing medications, including pain relief and symptom control, and issuing prescriptions as needed based on palliative care principles. Ordering and interpreting diagnostic tests as necessary to inform the care plan while considering patient comfort and care goals. Coordinating with a multidisciplinary team (e.g., social workers, spiritual care, nurses) to provide holistic, patient-centered care and facilitate smooth transitions between care settings. Educating patients and families about healthcare options, emphasizing palliative care choices and advanced care planning.
