Job Description
Job Overview We are seeking a dedicated and compassionate Nurse Practitioner to join our Cardiology Specialty Clinic. In this vital role, you will provide advanced clinical care, perform comprehensive patient assessments, and develop personalized treatment plans in an office setting. Your expertise will help improve patient outcomes, support health education, and ensure high-quality care delivery. Responsibilities Conduct thorough physical examinations and patient assessments to determine health status and diagnose conditions. Manage acute and chronic illnesses through evidence-based treatment plans, including medication administration and diagnostic cardiac procedures. Collaborate with multidisciplinary teams utilizing EMR (Electronic Medical Records) systems like Tebra, Epic, Cerner, or eClinicalWorks to document patient encounters accurately. Provide telehealth consultations to expand access to care, ensuring compliance with HIPAA regulations and maintaining high standards of patient confidentiality. Contribute to case management efforts, and support discharge follow up care for hospitalized patients. Educate patients on health promotion, disease prevention, nutrition, and medication adherence while working with diverse populations including geriatrics, behavioral health, and individuals with disabilities. Qualifications Valid Nurse Practitioner license with current certification in advanced practice nursing. Proven experience in critical care environments such as ICU or Level I Trauma Centers is highly desirable. Strong background in medical office procedures including CPT coding, ICD-10/ICD-9 coding systems, and medical documentation standards. Hands-on experience with EMR/EHR systems like Epic or Athenahealth; telehealth experience is a plus. Knowledge of specialties including gastroenterology, hospice & palliative medicine, occupational health, emergency medicine, internal medicine, geriatrics, pediatrics (including neonatal ventilator management), and behavioral health are a plus. Ability to work effectively in fast-paced clinical setting while demonstrating excellent patient care skills.
