Nurse Practitioner Hospice

Providence Hospice Oregon is seeking a full-time, certified nurse practitioner in Adult, Family, Acute or Geriatric Medicine to join a skilled interdisciplinary team providing coordinated, comprehensive Hospice care to patients and families in the Portland, Oregon metro area. The nurse practitioner in this role will be providing emergency assessments, recertifications, symptom management, educational and evaluative services, assists patients and families with goal setting related to advanced care planning and end-of-life process. Ideal candidates will be advanced Hospice and Palliative certification or willing to pursue and have a strong interest in end-of-life care. Providence Hospice Oregon is part of a growing integrated Home and Community Services program which collaborates with Home Health, Palliative Medicine, Geriatrics as well as hospital-based practitioners to care for patients and their families. Full-time Monday to Friday, daytime hours Outpatient, community-based setting; may do virtual visits occasionally Call 1 weekday per week and 1 weekend per month Ideal candidates will have advanced Hospice and Palliative certification or willing to pursue and have a strong interest in end-of-life care Competitive salary, quality incentive, CME funds and hours, and relocation assistance available.
