Key Responsibilities:
Provide on-site wound care and patient rounding services. Manage pressure injuries, ulcers, post-surgical wounds, and complex cases. Perform assessments, treatment planning, and follow-up care. Collaborate with physicians, PAs, nursing staff, and interdisciplinary teams. Support proactive care initiatives that reduce hospitalizations and improve outcomes.
Qualifications:
Active Minnesota Nurse Practitioner license in good standing. Wound care, geriatrics, post-acute care, SNF, or primary care experience preferred. Strong clinical judgment and comfort managing complex patient populations. Valid driver's license and reliable transportation required.
