Key Responsibilities:

Provide advanced wound care and mobile rounding services. Manage pressure injuries, ulcers, and complex wound cases. Conduct assessments, medication management, and follow-up visits. Collaborate with physicians, nursing teams, and interdisciplinary staff. Support continuity of care and improve patient outcomes.

Qualifications:

Active Minnesota Nurse Practitioner license in good standing. Experience in wound care, geriatrics, post-acute care, SNF, or primary care preferred. Strong clinical judgment and comfort with complex patients. Valid driver's license and reliable transportation required.
