About the Role We're seeking a compassionate Nurse Practitioner to join our Program of All-Inclusive Care for the Elderly (PACE) team. In this role, you'll provide primary care and chronic disease management for older adults in a collaborative, interdisciplinary environment. What You'll Do Deliver primary, acute, and chronic care to PACE participants in clinic, home, and facility settings. Works in collaboration with the clinical team and under the direction of the Medical Director. Complete comprehensive assessments and develop individualized care plans. Manage medications, order/interpret diagnostics, and coordinate care transitions. Participate in Interdisciplinary Team (IDT) meetings to ensure integrated, person-centered care. Educate participants and families on health conditions, prevention, and wellness. Document care accurately in the EHR and support quality improvement initiatives. What We're Looking For Master's or Doctorate in Nursing; active NP license. Adult, Family, or Gerontology NP certification preferred. Experience in geriatrics, primary care, long-term care, or chronic disease management. Strong clinical judgment, communication, and teamwork skills.
