$130,000•$150,000 + RVU Bonus Opportunities About the Opportunity Genesis Medical Health is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our team providing primary care services to geriatric patients in a skilled nursing facility (SNF) located in the Canarsie section of Brooklyn, NY. This role focuses on delivering high-quality medical care within the nursing home setting. The provider will manage a panel of patients, coordinate care with facility staff, and ensure patients receive comprehensive and compassionate medical care. Schedule The provider will work Monday through Friday with flexible scheduling. Providers may choose their daily working hours (for example 8-3, 9-5, or similar). Providers must remain available and accessible by phone between 8:00 AM and 6:00 PM for patient care needs. Compensation Structure Base salary between $130,000•$150,000 Additional RVU-based productivity bonuses paid quarterly Option for fully RVU-based compensation for higher earning potential Option to work as either W2 or 1099 Benefits Health insurance (single plan) Dental and vision coverage Malpractice coverage DEA reimbursement Four weeks PTO Clinical Responsibilities Provide primary care services to geriatric patients in a nursing home Perform routine visits, follow-ups, and chronic disease management Coordinate care with nursing staff and facility administration Participate in care management programs including CCM and RPM Document patient encounters in facility EMR systems such as SigmaCare or PCC Requirements Active New York NP or PA license Active Medicare and Medicaid provider ID Experience working with geriatric patients preferred Skilled Nursing Facility (SNF) experience preferred RN background in a nursing home or long-term care setting is highly valued for NP candidates Providers must be comfortable working independently within the nursing home environment About Genesis Medical Health Genesis Medical Health is a growing medical group focused on providing high-quality primary care services in nursing homes and long-term care facilities.
